The Care Coordinator is responsible for creating and maintaining an appropriate linkage system within the Health Center and various community agencies. The Care Coordinator advocates on behalf of the BHS clients, and assists in engaging clients? follow-through with their treatment plan.
Essential Duties and Responsibilities
- Coordinates referrals with BHS Screener and internal Providers in a timely and professional manner.
- Follows-up with referral sources per set standards.
- Promotes coordination of service delivery and prevents duplication of services.
- Follows-up on results of services provided to clients per set standards.
- Participates in discharge planning as needed.
- Meets productivity standards and performs duties as workload necessitates.
- Participates in the weekly Inter-disciplinary Treatment Team.
- Collaboration with BHS Clinicians around patient needs.
- Maintains client confidentiality per state and HIPPA laws.
- Attends Provider?s meetings in the community to foster collaborative relationships.
- Completes applications for basic needs, transportation, housing and treatment programs in a timely manner.
- Coordinates with community agencies for outreach and engagement: court personnel, parole and probation, discharge case managers from hospitals and treatment centers.
- Ensures to establish and maintain good working relationships with community agencies.
- Attends and participates in multi-disciplinary treatment team meetings.
Qualifications:
- Education: Bachelor?s degree in human services related field.
- Experience: At least two years of Human Services/case management experience is preferred. Excellent working knowledge of community social and health service agencies is preferred.
- Knowledge/Abilities: Excellent organizational skills. Knowledge and skills related to individual, group and family system dynamics. Knowledge of and demonstration of professional ethics. Ability to work well with multi-disciplinary service providers. Demonstrate age specific/population specific competencies in area(s) of practice. Knowledge and skills related to individual, group and family system dynamics. Proficient computer skills. Must demonstrate cultural competence. Candidates Bi-Lingual in English and Spanish strongly desired.
