Provide education, engagement, and long term follow up of those at risk of acquiring HIV through drug using or sexual partners of individuals with known HIV+ status or unknown HIV status. Monitor lab results needed to continue on PrEP, and support patients who are on PrEP at the Lowell Community Health Center per the standards and guidance of the Massachusetts Department of Public Health and the Office of HIV/AIDS (MDPH/OHA). Facilitates access to PEP (post exposure prophylaxis) as needed. Provide outreach/in-reach and recruitment as regulated by MDPH/OHA. (Includes offsite outreach).

-    Facilitate and track supported referrals.
-    Compete office related tasks such as answering phones and scheduling appointments as assigned by team lead.
-    Collaborate with other testing counselors in prioritizing and meeting patient needs.
-    Provide HIV, STI and HCV counseling and testing per the standards of MDPH/OHA.
-    Collaborate with nurse in the treatment and vaccination of patients.
-    Demonstrate proficiency in phlebotomy.
-    Provide overdose prevention and education.
-    Document services provided in patient?s EMR.
-    Complete all required MDPH documentation.
-    Complete monthly service delivery report.
-    Participate in monthly individual administrative supervision.
-    Participate in monthly group clinical supervision.
-    Provide supported referrals for Hepatitis A & B vaccinations.

Requirements
-    High School diploma or equivalent.
-    One year of community health/public health experience preferred.
-    Proficiency in phlebotomy preferred.
-    Ability to read and write in English required. Bilingual Spanish/Khmer/Portuguese preferred.
