Summary
The HHA is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient's needs. The HHA is responsible for observing patients, reporting these observations, and documenting observations and care performed.
The HHA will be assigned in a manner that promotes quality, continuity, and safety of a patient's care.
Essential Job Functions/Responsibilities
Responsibilities of the home health aide include, but are not limited to, the following:
- Providing personal care including but not limited to:
o Baths
o Back rubs
o Oral hygiene
o Shampoos
o Changing bed linen.
o Assisting patients with dressing and undressing
o Skin care to prevent breakdown.
o Assisting the patient with toileting activities.
o Keeping patient?s living area clean and orderly, as appropriate.
- Planning and preparing nutritious meals.
- Assisting in feeding the patient, if necessary.
- Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood
pressure when ordered (with appropriate completed/demonstrated skills competency).
- Assisting in ambulation and provides therapeutic exercises as prescribed by therapist.
- Performing range of motion and other simple procedures as an extensional therapy service as
ordered (with appropriate completed/demonstrated skills competency).
- Assisting patient in the self-administration of medication.
- Doing patient?s laundry, as appropriate.
- Meeting safety needs of patients and using equipment safely and properly (foot stools,
side rails, etc.).
- Provides any specialized care as specifically instructed by clinician.
- Reports and documents on CellTrak changes in patient status or expectations of service to a
supervisor, community health nurse or therapist.
- Provides basic emotional support to the patient and reports changes in the patient's mental
status to appropriate professional staff.
- Maintains a clean, safe, and healthy environment, within the patient?s home.
- Attends and participates in patient care conferences.
- Attends office meetings and trainings as required.
- Possesses and maintains a current CPR certification.
- Other duties as assigned.

Qualifications
- Educational: Successful completion of a nurse aide course that meets federal/state
requirements.
- Successful completion of the written competency exam and the skills demonstration evaluation
required for home health aide certification by the agency.
- High school graduate / GED diploma preferred.
- Licensed Nursing Assistant (LNA).
- Experience: Six months or more preferred.
