Description

Community Health Connections is a full-service community health center with four locations in Central Massachusetts. Our vision is to be the leading healthcare center, committed to eliminating barriers to care and building a healthier tomorrow for the communities we serve.
We are seeking a qualified Advance Practice Nurse to lead and provide care in our office-based Medication Assisted Treatment (MAT) Clinic, and provide prevention, screening, intervention and treatment related to substance use disorders.
Minimum Qualifications:
Massachusetts Licensed Registered Nurse required
Certified in Massachusetts to practice in the expanded role as a Family Nurse Practitioner required
Current active, unencumbered DATA2000 Waiver to allow for prescribing MAT
CARN-AP preferred
Mission-driven and motivated to join an organization that will transform the way we deliver accessible, evidence-based and clinically excellent care in the substance use disorder space
Ability to work across teams and functions
Self-starter who quickly identifies necessary actions and takes ownership
Certified in CPR/AED or equivalent
Demonstrated mastery of a specialty area or other related field normally acquired by 3-5 years progressively responsible nursing experience preferred
