MassHire Lowell Career Center has an opening for a Community Health Worker for a well-known Community Health Center in the  Greater Lowell area.

Job Title: REACH LoWELL COVID/Flu - Community Health Worker

Schedule: Grant funded. Full-time (40 Hours/week). Non-Exempt. Some evenings and weekends required.


Job description and responsibilities
Responsibilities include the following  and other duties may be assigned.
- Conduct extensive outreach in their community, with the goal of enhancing awareness of resources, more so related to COVID/Flu vaccine, education and awareness
- Work collaboratively with REACH LoWELL COVID/Flu Coordinator to support program deliverables
- Provide translation and interpretation support in target language in alignment with REACH LoWELL program deliverables
- Promote health education in alignment with REACH LoWELL COVID/Flu strategies
- Be knowledgeable about community resources appropriate to the needs of patients/families and provide services such as arranging transportation, referrals to community programs, setting up appointments when needed and connect to financial support
- Collaborate with community agencies to coordinate resources/referrals and network with community leaders
- Support patients and their families to navigate and access community services, other resources, and adopt healthy behaviors
- Attend and actively participate in appropriate trainings and meetings relevant to REACH LoWELL COVID/Flu strategies
- Provide advocacy for initiative priorities through a strong presence at community meetings, events and activities.
- Participate at various community events and Community Health education efforts, including community screenings, Community education groups and Health Center?s Advisory groups, as needed.
- Represent Health Center mission and programs at meetings, conferences, on the phone and in person
- Prepare reports in accordance with Health Center procedures and ensure that the reports are submitted in a timely manner as required by REACH LoWELL COVID/Flu Program Director and participate in staff meetings
- Assist with required data collection and evaluation reporting as directed.
- Provide evaluation support to REACH LoWELL COVID/Flu team
- Reports to Health Center REACH LoWELL COVID/Flu Outreach Coordinator
- Perform other duties as requested to support the mission of REACH LoWELL and Health Center
- Maintain standards that ensure quality of care in the delivery of program services and compliance with state and federal guidelines. Grant funded position, available pending grant availability.

Qualifications and Requirements
- Lives in or familiarity with the target community and feels connected to that community
- Community organizing skills
- Excellent verbal and written communication skills
- Strong presentation skills
- Two to four years? experience in a community based setting or health related position
- Demonstrated experience working with diverse cultures
- Detail oriented, with strong organizational skills and multi-tasking abilities
- Ability to work independently with minimal supervision and as part of a team
- Experience working with grant funded programs
- Must have reliable transportation
- Proficient computer skills in MS Office

Preferred Education and Training
- Community Health Worker Training Certificate
- At least 2 years education in Public Health, or equivalent
- Knowledge of the US health care system and medical terminology
- Ability to read and write in Spanish

LANGUAGE SKILLS: Bilingual in English and Spanish is required.
